I could do that but the problem is that I have some machines, for example, that may be running a full version of Office and only Access so if I apply a SP or security update for to that machine it will error out. So I have to separate those types of machines into different groups and pay close attention to what I apply and where. If adding approvals to additional groups resulted in the successful installation of the update, then it wasn't directly the case that all groups need approvals, but more likely that the computer wasn't actually a member of the original group where the first approval was issued.
